Title: “Care Plans – Fact or Fiction?” – REPEAT BY DEMAND
Biography: Lisa worked in her small, hometown facility throughout high school and returned 25 years later to take over as the Activity Director. She spent five years in the facility and during that time Lisa became nationally certified thru NAAPCC and NCCAP, obtained her CDP certification and received her Consultant certification which led to her opening her own business. Lisa works with the KS Task Force to reduce anti-psychotics in dementia patients. And in 2014 she became certified in Alzheimer’s Disease & Dementia Care training. Lisa enjoys her role as Education Outreach Trustee with NAAP and speaking at state and national conferences.

Objectives:
- To better understand requirements for assessing and writing Care Plans
- To discuss Resident’s Rights and the Federal Regulations required for Care Plans
- To educate Activity Professionals on effectively writing Admission Assessments
Program Description: Care Plans seem tend to be one of the harder duties of the Activity Professional. In this session Lisa would like to talk about the Admission Assessment and the information that is so critical for us to obtain to insure that we write an appropriate Care Plan. She will discuss Progress Notes, Resident Rights, the Federal Regulations relative to Activities, and the CAT’s and CAA’s. Also, Lisa will be sharing some very helpful resources that she has found to assist with Care Plans.
